How IVF Due Dates Are Calculated

IVF due dates are determined differently from naturally conceived pregnancies. In a standard pregnancy, the due date is calculated by adding 280 days (40 weeks) to the first day of the last menstrual period (LMP). With IVF, the exact age of the embryo at the time of transfer is known, which allows for a more precise calculation.

Day 3 Embryo Transfer

When a Day 3 embryo is transferred, the embryo is already 3 days past fertilization (conception). Therefore, the due date is calculated by adding 263 days to the transfer date (266 days from conception minus the 3 days the embryo has already developed).

Day 5 Blastocyst Transfer

A blastocyst transfer occurs on Day 5 of embryo development. The due date is calculated by adding 261 days to the transfer date (266 days minus 5 days already elapsed).

Egg Retrieval Date

If you know only the egg retrieval date, this is treated as the day of fertilization (conception day). The due date is calculated by adding 266 days to the retrieval date.

Gestational Age vs. Fetal Age

Gestational age is counted from the last menstrual period (LMP), which is approximately 14 days before ovulation/fertilization. Fetal age (also called embryonic age) is counted from the actual day of conception. For IVF pregnancies, gestational age is calculated by adding 14 days to the fetal age.

Fresh vs. Frozen Embryo Transfers

The calculation is the same whether you had a fresh or frozen embryo transfer — what matters is the developmental stage (Day 3 or Day 5) of the embryo at the time of transfer. Both fresh and frozen transfers use the same formula.

Trimesters of Pregnancy

  • 1st Trimester: Weeks 1–12 (0–84 gestational days)
  • 2nd Trimester: Weeks 13–26 (85–188 gestational days)
